Just complete the form with the old invoice number and the names and text for the new order. If you don't have the previous invoice or order number, just let us know what the item was, and we can find it. If you prefer to send an email with your information to service@awards123.com that would be great too - one of our highly trained staff will look up your previous order and process your new one.
